Initial Eligibility
Each month the Contributing Employers send a report to the Plan Administrator listing who worked for them in the previous month and the hours worked by each member with the payment for Contributions.
The Plan administrator sets up a Dollar Bank Account for each person, depositing to that account all of the contributions earned and received by the Plan Administrator. Provided that you are a member in good standing of LiUNA Local 493, you and your eligible dependents will be covered by the welfare plan on the first day of the second calendar month following the month in which your contributions have reached $250.
For each month that you are covered by the Welfare Plan, $250.00 will be deducted from your own dollar bank account in order to pay for your benefits.
Reinstatement
If your coverage has previously terminated because of insufficient dollars in your dollar bank account or due to administrative suspension for not meeting the requirements of the member in good standing of LiUNA Local 493, you will again become insured on the first day of the second month following the accumulation of 1 month of coverage costs in your dollar bank account or reinstatement as a member in good standing.
